I expressly agree that REPORTING.CZ s.r.o, with its registered office at Sokolovská 131/86, Praha 8, 186 00, ID: 24248991, as the Controller, manually process my personal data, namely name, surname, electronic (e-mail) address and telephone number, in connection with the download and use of the Addin “REPORTING.CZ FOR EXCEL” and in connection with the provision of services in the form of the Administrator (Administrator) individual communication, not within the meaning of Act No. 480/2004 Coll., on certain information society services, as amended from 1 July 2017).

I give my consent within the meaning of Article 6 (1) (a) of REGULATION (EU) 2016/679 OF THE EUROPEAN PARLIAMENT AND OF THE COUNCIL of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) and declare that all data provided by me is true.

I give my consent to the above-mentioned processing of my personal data voluntarily, for a period of 10 years from the date of its provision.
